How to make a request for information

Informal Process

In certain circumstances, it may not be necessary to submit a formal request. If the information you are requesting would normally be released, you may request the information through an informal process. Requests can be directed to the Waypoint FIPPA coordinator by describing the records you are requesting. The FIPPA coordinator will let you know whether the information will be released or if you are required to submit a formal request under the Freedom of Information and Protection of Privacy Act (FIPPA).

Formal Process

When an informal request for information is denied, a formal application can be made under FIPPA. Applications must be in writing on the application form provided and submitted with the mandatory $5.00 application fee to Waypoint's FIPPA Coordinator. 

Please provide as much detail as possible to help us identify the records. The more specific your request, the more efficiently and accurately it can be answered. The FIPPA Coordinator may contact you to clarify your request.

We will make every reasonable effort to respond to a request no later than 30 calendar days after receiving your request, unless specific exceptions apply that warrant an extended period of time to respond.

How to make a request


1.    Check the Directory of Records to make sure we maintain the information you need.

2.    Download and complete the FIPPA Application for Access Request form, clearly identifying the records
       you are seeking. Be sure to include your contact information.

3.    Include a cheque or money order for $5 payable to "Waypoint Centre for Mental Health Care".

4.    Submit your request, together with the $5 non-refundable application fee to:
        FIPPA Coordinator
        Waypoint Centre for Mental Health Care
        500 Church Street
        Penetanguishene, ON L9M 1G3

Once your application is received, we will send written acknowledgement of your request. If you need help completing the form, contact Waypoint's Access and Privacy Officer at 705 549-3181, ext. 2258.

Appeal Procedure

Under the Act, you may appeal any decision we made regarding access or correction to information. Appeals must be made to the Ontario Information and Privacy Commissioner within 30 days from the date you received the letter denying your request.
