Geriatric Services Program
The Geriatric Services Program is comprised of three services: the inpatient assessment and treatment unit, a community support team (CST) which offers outreach in the community, and a Day Clinic which serves both inpatients and community clients. All three components provide psychiatric services to persons within our service area who are over 65 years of age and presenting with serious psychiatric illness, dementia, and/or challenging aggressive behaviours.
Inpatient Program
The GSP inpatient program is a 26 bed unit that provides multidisciplinary team assessment, treatment and discharge planning for older adults meeting the above mentioned criteria. We offer assistance in meeting all basic care needs, supportive counselling to facilitate recovery, and we implement quality of life improvements that support strategic care interventions. Admission is usually for no longer than three months.
The multidisciplinary team is comprised of a variety of specialized healthcare professionals including psychiatrists, social workers, occupational therapists, and rehabilitation support staff who work together with other hospital and community services as needed.
Home Visit Geriatric Program
Our Community Support Team is dedicated to providing psychiatric outreach assessment and treatment recommendations for the older population. Services are provided by a team of RNs, psychiatrists and a community support worker.
Our CST offers assessment of care needs, support to facilitate recovery, and suggestions for strategic interventions to reduce/ mitigate stigma and symptoms, as well as provides treatment recommendations on a consultative basis. Patients are assessed in a comprehensive manner, using measurable outcomes and individualized consultations. Care recommendations are based on patient goals and follow-up is arranged based on the complexity of care issues.
Day Clinic
The GSP Day Clinic offers assistance to both inpatients and community clients in developing self care strategies, supportive counselling to facilitate recovery, and implementation of quality of life improvements utilizing strategic care interventions such as therapeutic support/rehabilitation. Care provision is based on patient goals throughout the assessment period.
The program partners with community resources using a referral process that facilitates external supports evenings and weekends.
Patients requiring any of our three services require a physician or other healthcare professional to refer and referrals may come from long term care homes, hospitals or community agencies or other services for assessment, and will return to the care of the referring agency/physician once discharged.
