Outpatient Assessment and Treatment Service


The Outpatient Assessment and Treatment Service (OATS) provides a goal-directed service for individuals experiencing moderate to severe mental health challenges where care needs cannot be met through other community resources.  

The services through OATS are time limited (6-9 months) and may include psychiatric consultation, as well as group therapy and individualized supports for treatment and recovery. 

The hours of operation are Monday to Friday from 8 a.m. to 4 p.m. and we are located at the Community Health Hub at 287 Bayshore Drive, Midland. 

Services may include:
 * Psychiatric consultation, assessment, and treatment, including diagnostic clarification and
    medication stabilization

 * Medication therapy and monitoring
 * Evidence-informed psychotherapeutic and psychoeducational groups
 * Interprofessional support
 * Advocacy and connection to other community resources
 * Discharge planning

Program eligibility
Individuals 16 years and older experiencing moderate to severe mental health conditions.

Referrals
Referrals to OATS must be completed by a Primary Care Provider using the Waypoint Referral Form

Access to OATS
Following acceptance to the OATS program, a Clinician will contact the client directly to schedule an Intake Assessment. Once this is complete, clients will be provided with an appointment for consultation with a Physician. Clients will also start to work with a treatment team comprised of nurses, social workers, and occupational therapists to develop unique care plans to support their journey towards health, wellness, and recovery. Access to other care providers is available as needed including dieticians, spiritual care providers, and pharmacists. 

Information for Primary Care Providers

ONT eConsultseConsult Services are now available for Primary Care Providers via the Ontario eConsult Program though OTN Hub. eConsults are intended for specific questions about patient care (e.g., medication optimization) and allow for a more timely and convenient means of consulting with our Outpatient Specialists for non-emergent issues. You can now send questions about your patients, their medical history and their test results for review through a secure, online platform, enabling you to expedite access to specialist advice. eConsults will be responded to within seven business days.
