Road to Recovery program


The Road to Recovery program is a partnership between Waypoint and three community mental health housing providers in Simcoe Muskoka/Parry Sound. The program focuses on securing affordable, permanent and independent housing opportunities for Waypoint patients who have experienced frequent or lengthy psychiatric hospitalizations, and where the acquisition of suitable supported housing and comprehensive community supports is deemed critical to their recovery and community integration. The goal of the program is to enhance choice by increasing options for high support housing, thereby enabling health recovery, community integration and tenure, and an increase in quality of life.

To be eligible for this program, the individual:

   •    is a Waypoint in-patient without a fixed address
   •    has history of serious and persistent mental illness (addictions), with frequent or lengthy
         hospitalizations
   •     is psychiatrically stable and capable with intensive community supports in place
   •     is accepting of community mental health supports
   •     does not require 24 hour supervision
   •     is medically stable
   •     chooses to participate

To make a referral, please complete and submit an Outpatient referral to Central Intake. Please include supporting clinical documentation.
