Waypoint Centre for Mental Health Care and Bayshore HealthCare’s Integrated Care Solutions are proud to announce that Waypoint At Home has received the Canadian College of Health Leaders (CCHL) Solventum Health Care Innovation Team Award, recognizing innovative approaches that improve patient outcomes and transform how care is delivered.
For people living with mental health and addictions challenges, leaving the hospital can be one of the most vulnerable points in recovery. Without the right supports in place, individuals may struggle to maintain progress, leading to setbacks, crisis or avoidable returns to hospital.
Waypoint At Home was created to bridge that gap.
Developed through a partnership between Waypoint and Bayshore HealthCare, the program combines home care services with specialized mental health and addictions supports to help people recover safely and successfully in their communities. Planning for the transition from hospital to home begins while patients are admitted, with the care team working collaboratively with patients and their caregivers. Care plans are developed, tailored to the specific needs of each individual putting things in place as soon as patients return home.
The impact has been significant:
- 0% 30-day hospital readmissions
- 0% 30-day emergency department revisits
- 91% program completion rate
- 100% of surveyed participants would recommend the program
This recognition reflects what is possible when care extends beyond hospital walls. Waypoint At Home was designed to help people continue their recovery with the right support in place at the right time, giving them greater stability, confidence and connection as they return to everyday life.

The award comes as healthcare organizations across Ontario continue to explore new approaches that strengthen transitions from hospital to home, improve patient experience and reduce pressure on emergency departments and inpatient care.
