The Community Consultation Service – (CCS-DD) program is a specialized and time-limited physician consultation service for individuals who have a mental health diagnosis, in addition to a diagnosis of intellectual/developmental disability and/or with significant adaptive/behavioural challenges.
The main purpose of the CCS-DD program is to provide short-term comprehensive physician consultation focusing on pharmacological management and the creation of recommendations for ongoing support in the community, in collaboration with primary care providers and other community-based organizations and support services (e.g. Developmental Services Ontario). Nursing support is also provided to monitor and support the implementation of recommendations and the treatment plan.
The hours of operation are Monday to Friday from 8 a.m. to 4 p.m. at the Community Health Hub (287 Bayshore Dr., Midland). Services are also available virtually or by telephone.
