Community Homes for Opportunity (CHO) is a recovery-oriented, person-centred supportive housing program for people with serious mental illness who may require 24-hour supportive care. The program is intended to assist tenants by providing appropriate housing and support services to achieve and maintain housing stability that is safe and affordable.

CMHA-Simcoe County is administering the CHO Program in Simcoe County.

To be eligible for this program, the individual:

• is a Waypoint in-patient without a fixed address
• has history of serious and persistent mental illness, with frequent or lengthy hospitalizations
• is psychiatrically stable and capable with intensive community supports in place
• is accepting of community mental health supports
• does not require 24 hour supervision
• is medically stable
• chooses to participate

To make a referral, please complete and submit a referral form to Central Intake. Please include supporting clinical documentation.

Clients or Providers can support clients by calling, texting, or connecting online with 211. 211 can be accessed 24 hours a day, 7 days a week.

At these access points, trained workers will use a common approach (including common intake forms and assessment tools) to assess the depth of need and help them complete the process that will prioritize them for housing support services. Clients will be matched to housing focused interventions in the community.

Domiciliary Care Facilities are privately owned residential facilities that provide shelter, food and supervision to vulnerable seniors and persons with special needs. The County of Simcoe funds beds in approved Domiciliary Care facilities within the County in order to cover the costs of shelter and personal needs for low-income individuals who cannot live on their own but do not qualify for long term care. Contracted domiciliary care providers are located throughout Simcoe County.​
