Current and former patients, family members and caregivers are invited to share their experiences through a short survey about outpatient and community programs and services. Feedback will help identify what is working well and where improvements could make services easier to access and better connected for patients and families.
Who can take part?
The survey is open to:
- Current or former Waypoint patients
- Family members of a Waypoint patient
- Caregivers of a Waypoint patient
- People connected with Waypoint’s outpatient and community programs and services
The survey takes approximately 10–15 minutes to complete.
Participation is voluntary, and choosing not to participate will not affect current or future services.
Complete the survey for a chance to win a $50 gift card. Three winners will be selected after the survey closes.
