Depending on individual needs, HART Hub services can connect people with:

• Comprehensive mental health and addictions assessment
• Psychiatric assessment and recommendations
• Individual concurrent disorders counselling
• Group/individual treatment and psychoeducation
• Withdrawal management and addiction treatment
• Primary care coordination
• Housing and supportive housing services
• Case management and system navigation
• Employment and social supports
• Indigenous-specific, culturally safe services

The HART Hub Navigation Team helps identify appropriate services and supports and assists with connections across the system.

Attend a Drop-In Session with the Navigation Team every Friday morning from 9:00 a.m. to 12:00 p.m. at 136 Bayfield Street.

Waypoint Centre for Mental Health Care provides specialized concurrent disorders services as part of the HART network.

Treatment is recovery-oriented, trauma-informed, evidence-based, and tailored to each individual’s goals. Services may include Cognitive Behavioural Therapy (CBT), Dialectical Behaviour Therapy (DBT)-informed interventions, Seeking Safety Group, motivational interviewing, relapse prevention, Family Psychoeducation and psychoeducation to support long-term recovery.

Clients can also be referred for a psychiatrist consult for diagnostic clarification as well as medication recommendations.

The Concurrent Disorders Program typically offers 5 different programs ranging from 5 weeks to 25 weeks in length, with treatment plans developed collaboratively and adjusted according to each person’s goals and progress.

Family Psychoeducation programming is 4 sessions at this time and is on a rotating basis. All family programming is offered virtually and free of charge.

HART Hub Simcoe County is intended for adults experiencing or at risk of homelessness who have complex health, mental health, addictions, housing or social needs.

This may include people who:

• Are experiencing homelessness or housing instability
• Are at risk of losing housing
• Are experiencing mental health or substance use concerns
• Have difficulty accessing health, mental health or addictions services
• Need support to find or maintain stable housing
• Have multiple needs requiring support from several service providers

Family members, caregivers and others supporting an individual may also contact the HART Hub for information about available supports.

HART Hub Simcoe County is a regional partnership involving healthcare, mental health, Indigenous and community organizations. Partner organizations work together to improve access to services and support connections between health, housing and community-based care.

Waypoint Centre for Mental Health Care is one of the partners involved in HART Hub Simcoe County.

Other partners include the County of Simcoe, Barrie and Area Ontario Health Team, Barrie Native Friendship Centre, Canadian Mental Health Association – Simcoe County Branch, Barrie Area Native Advisory Circle/Mamaway Wiidokdaadwin IIPCT, and Royal Victoria Regional Health Centre.
